Below are downloadable applications to attend a Flight of Hope Chrysalis Flight. The Flight of Hope Chrysalis Community asks that...

1. BE PRAYERFUL about sponsoring a youth for a Chrysalis Flight.
2. READ AND FOLLOW the Sponsor's Responsibilities sheet included with the application. The sheet has extremely important information regarding events that happen on the weekend and board procedures for sponsors.
3. BE PREPARED to follow up with the butterfly you sponsor after the flight; see the Sponsor's Responsibilities sheet for more details.
